For orders shipped withing Pennsylvania, we require a signed REV-1220 to be fax to 888-453-3544 to exempt sales tax.

Check "State or Local Sales and Use Tax" at the Top
Check "Pennsylvania State Blanket Exemption Certificate" (We will Keep this on File for Future Use)
You May Leave the Seller Information Blank, we will fill that out.
Check Box #4, Put "Volunteer Firemen's Organization" and your 75-xxxxx-x Exemption Number in the Blanks
Fill Out the Organization Name, Address, EIN, & Date at the Bottom of the Form.
Print and Sign the Form
